Your doctor will be able to complete most of the paperwork you will need at the time of your visit. On occasion, you may require special forms and reports to be filled out by your physician, either at the time of your visit or as a follow-up. (If you require additional paperwork after your visit, we ask that you please drop it off to us and we will contact you when it is ready for pick-up.) We do charge an appropriate fee for this service, since the time utilized in completing some forms can be extensive. This fee is based upon the complexity of the paperwork; we are happy to provide you with the amount of the fee in advance. Insurance companies will not cover this expense.

Reports to attorneys and other administrative agencies are also labor intensive. Please ask about the fee for this service prior to requesting that a detailed report be prepared on your behalf.
Copies of medical records can be provided when necessary, and we are asked to prepare these for a variety of reasons. Often, obtaining copies of office notes and test results is much less expensive than preparation of formal reports, and accomplish the same goals as detailed medical summaries. We utilize the state of NC and SC statutes in determining the photocopying charge for medical records. There is no charge for copies of necessary medical records for use by specialists when you are referred by one of our physicians. In Workers' Compensation cases, your employer and their Workers' Compensation carrier may receive your records at no charge.
If you would like to obtain a copy of your medical records, it is necessary to fill out a medical release. You may stop by and pick one up or print one out from the forms section.
Record Search Fee:
$15.00 initial fee for chart search
$.65 per page for pages 1 - 30
$0.50 per page for pages 31 or more
(Plus postage if mailed)
It is our policy to receive payment in advance for copies of reports, paperwork and medical records. We will mail the records via First Class mail within two to four weeks of receipt of payment or they may be picked up at one of our facilities. If you require an expedited form of delivery, there may be additional charges associated with that service.
